Rationale and Costs –Immediate Same Day Extraction and Back Tooth Molar Implant vs.Waiting

Back molar teeth can have immediate implants!  This means that the same day a molar is extracted, an implant can be put in the socket.  When it is possible, this saves you money by avoiding the cost of a second procedure or bone graft.  Besides cost, it also saves you time because you don’t have to “extract, then let it heal for 2-4 months.”  You want your tooth as soon as possible!

In order to do an immediate implant for a back molar tooth a large, wide diameter implant must be used.

Super Wide Diameter Implants (6.0mm and larger)
-reduces or eliminates the need for bone grafting in back tooth extraction sites

Considerations
-Size of tooth being replaced.  The average measurements of back teeth indicate use of a 7.0mm implant. Tooth size varies by type of tooth as well as your genetics

-Nerve position is critical

Increased initial stability (tightness when first placed)
-When the diameter of the root is less than that of the implant, the resulting primary stability is greater. This is important for the


long-term of your implant.
-tapered implant body design is suited to the mandible’s morphology and allows placement of a longer implant for greater surface area
-Reduction (or elimination) of the need for bone grafting!

Benefits for the Crown Part of the Implant
-Increased surface area by the use of a big implant
-Reduces the force applied to the implant-bone junction
This in turn, reduces the risk of bone loss and subsequent implant failure
-Reduces the stress delivered to the rest of the implant
-Increases the strength of the implant and reduces risk of fracture

A 7.0mm diameter implant is approximately 16 times stronger than a 3.5mm diameter implant

This is a procedure I have done for 14 years. I say this because it seems to be only recently that others have caught on to this great procedure!

Having a same day molar implant has to be planned very carefully and can be used for 10-30% of situations. Although reducing cost and time is important, safety comes first.

  1. I,had two attempts at titanium implants both failed a zirconium worked for last 5 Year’s.family hx of metal allergies. Number 14 molar fractured can you do same day implant with total zirconium either one piece or two piece zirconium and extraction. Immediate crown not critical to me. Expected cost and note I’m in needles calif five hours away. Age 66

    I have researched professionals like yourself who can do an extraction and implant in one setting. It makes sense to have such a course of treatment.

    my tooth #17 is falling apart after many years and should be extracted. However, I am reluctant to extract without a plan of action to replace the empty space which will be created. Position #18 is empty and was bridged between #17 and #19. # 19 is in good order. I have dental coverage for the extraction and a new bridge if that becomes the recommended course of action. The cost of the implant is not covered by any insurance that I have or know of.

    I am 67 and usually in great health, my fitness and appearance always surprise doctors. Two weeks ago I had a Open Heart procedure to repair my Mitrol valve which went well. I was discharged from the hospital in 3 1/2 days.

    What is your opinion relative to me and my pursuit of a implant and new bridge with an extraction and implant setting the same day?

    • Hi Mike,

      If 18 is already missing, then extracting 17 and placing 18 on the same day is the way to go! Keep in mind that the space of 18 may be narrow and need a bone graft because the tooth has been missing for some time.

  3. Hi Jack,
    If you have upper teeth above it, it is far better to have 2-3 implants. This will support your bone and prevent bite collapse.
    Avoid “minimal engineering” for your dental implant restorations.
    Bone grafts can routinely be done on the same day that the implants are placed.

  4. I have been missing teeth number 17,18 & 19 for over 40 years. Periodontist said that he can do an implant where tooth 19 is but that I don’t need anymore implants than that to function. He will graft it that day and put the implant in there the same day. From reading your material is this really wise to do in one day? Can I function with just one implant? There are teeth on the top above these? thank you.

  5. Hello Dr Ramsey,
    I am a very healthy 52 yr old female(if that matters) Non smoker, No Medicines (except for antibiotics during implants) exercise eat healthy… My Oral maxafacial surgeon has been unsuccessful in completing right back molar implant. twice failed. He is recommending to remove failed implant and replace immediately with larger implant (if site looks healthy enough) Is there any cautions you may have for me going forward? Would you recommend I seek another opinion and from what kind of surgeon? I am in New Jersey, 07090 zip code, if you have any recommendations/referrals I greatly appreciate your time and concern.
